COMPONANTS OF A PERSONALIZED HOLISTIC CARE PLAN
To identify the root of a problem, a health history must be obtained. Often times the problem being experienced is the fruit of a much deeper problem. The depth of the problem could even go to the emotional or even spiritual level. This is one of the reason the collection of information must be viewed in a holistic way.
The holistic approach takes into consideration the whole person. Mental state and social factors as family history, experiences and relationships are included in the data collection. Childhood trauma can have a lasting effect into adulthood, which can profoundly impact health.
In planning how to meet the needs of the client, this must be considered in a holistic way. For the plan to be successful the client or caretaker must have the ability to fulfill it. Learning readiness must also be assessed for client or caretaker. It may be necessary to divide the plan into manageable parts to prevent the client or caretaker from feeling overwhelmed. The goal of the plan should be set with the client and agreed upon whenever possible.
In the implementation portion the client or caretaker caries out the plan. This is written clearly and in details. A step by step process can be followed more easily and makes it easier to measure the results. The supports to improve physical health are specified in this part of the plan. Choices related to physical health are also made known.
In the evaluation portion of the plan the results are analyzed and measured. Based on the results it can
be determination if progress is being made, and the further course of action to be done. The client is an active participant in the plan and should be included in the process of setting the goal of the plan when possible.
